Overview

Bringing fragmented health information into one continuous patient record.

SMF Med gives patients one place to organise reports, prescriptions and health information, and gives healthcare providers controlled access to the context they need for review. We built a connected patient, provider and admin ecosystem, with AI-assisted report intelligence that makes health information easier to store, share, understand and follow over time.

2Keys to open a shared report: Report ID and OTP
7Stages from health profile to ongoing history

SMF Med case studyOverview
The industry problem

A patient's medical history rarely lives in one place.

When a patient sees a new provider, much of their context has to be rebuilt by hand. The latest report arrives without the previous one, and allergies or blood group get explained again and again.

The problem isn't just storing records digitally. It's having the right information, with context, when someone needs to review it.

The challenge

More than another medical document repository.

The system had to create continuity across the whole journey, while meeting three very different sets of needs.

1Health profile
2Medical record
3Report sharing
4Provider review
5AI insight
6Professional feedback
7Ongoing history

Our approach

Designed around the patient record, not isolated documents.

A report means more when it's read alongside the person it belongs to and the reports that came before it. So every upload joins a longitudinal record, the foundation for both provider review and AI comparison over time.

Capturing reports

From paper report to structured data in a few taps.

Patients file reports by category, such as diabetes, blood pressure or surgery, then scan them with their phone. The key report data is extracted and kept next to the original file.

Patient-controlled sharing

Portable health records, not public ones.

Instead of sending files through messaging apps or email, a report is shared through a deliberate access step, and the review happens inside the platform.

AI-assisted report intelligence

Faster understanding, without replacing professional review.

Reports are long, and seeing change means comparing against earlier results. AI takes on the manual information processing that comes before review.

1

Summarise

Surface what matters in a report, in a structured form.

2

Extract

Pull out the relevant report data for easier review.

3

Compare over time

Place newer and earlier reports side by side.

4

Highlight change

Make trends and differences easy to spot and investigate.

The AI supports understanding. Clinical interpretation and advice stay with the healthcare provider.

From single reports to health progress

A report is a snapshot. A sequence of reports is a history.

Where earlier records exist, the latest report is read alongside them, so nobody has to rebuild the history from separate documents.

Multi-provider collaboration

A patient's care rarely belongs to one provider.

Several providers can add comments and advice to the same record, so the history stays connected even as the people reviewing it change.

Patient visibility

A clear view of their own health information.

Records are usually made for professionals first. SMF Med gives patients a lasting place to return to their reports, profile details, AI summaries and provider feedback, so nothing depends on memory or lost files.
